FILL IN AND SIGN A MEMBERSHIP FORM
We will ask you to complete a membership form and sign up to our terms and conditions. To save time, you can download our New Membership Form, print it and complete it before you come in. Your details will be held on our computer system, but we won’t share them with anyone else.
PROVIDE US WITH TWO FORMS OF IDENTIFICATION 1) Photo ID: Passport, Bus Pass, Blue Badge
2) Proof of Address: Utility Bill or Bank Statement
Or A Driving Licence, Which Already Has Both
Deposits are refundable when the Wheelchair or Scooter is returned in its original condition
TOWN HIRE
Town Hire is only valid during shop opening hours. £5.00 covers Mon-Fri period of first visit and cannot be be carried over. Every Monday a new hire period begin. Scooters and wheelchairs to be return by 3:45pm.
